Thank you for visiting the community empowerment project map. In order for your organisation and project details to be included on the map, please take a few moments to complete this questionnaire.
This questionnaire should not take long to complete for individual projects. The questionnaire allows you to save the information you have entered after each section, allowing you to close the form at any time and come back to it later. Once you have completed the form please click submit, your information will then be checked by the project co-ordinator and published to the interactive map.
Please be aware that if you register your organisation you will be recorded as the primary contact. Anyone within your organisation who wishes to add any projects will need to contact yourself to do so. If there is someone else who is best placed to be the primary contact please consider asking them to complete the form.
